OMCC Anglesey Event Indemnity Forms

To save time on the morning of the event, if possible could you print out and complete TWO of the attached forms, one for each day.
DO NOT SIGN THEM YET, this needs to be done at sign in on the desk when you are issued your wristband and sticker.
There will be forms available on the day but pre-completing will save a lot of time.
